Specialized health advocates who better understand where you are in your journey provide a curated, personal experience. Specialized navigation support is offered in the areas of Diabetes Support, Oncology Support, Gender Affirmation Support, Caregiver Support, Legacy Planning and Social Workers.

Diabetes Prevention and Weight Loss – Omada

For eligible employees at risk for Type 2 diabetes and heart disease, Omada is a digital lifestyle program that inspires health habits you can live with long term. It combines the science of behavior change with unwavering personal support to help you make changes that actually stick. The program includes access to a health coach, a wireless scale, weekly online sessions, and other participants who may help offer real-time support. High Blood Pressure Management –  Livongo

The Livongo hypertension program helps make it easier to live with high blood pressure.

The program includes:

  • Connected Blood Pressure Monitor: Automatically uploads your blood pressure readings to your secure online account; the app provides valuable guidance and tips.
  • Support from Coaches When You Need It: Communicate with coaches about high blood pressure and get answers to all of your questions on nutrition and making lifestyle changes.

Musculoskeletal Pain – Hinge Health

Hinge Health is a coach-led digital program for chronic back, knee, or hip pain. The program provides exercise therapy using wearable motion sensors and a free computer tablet for guided exercise therapy. You also have access to a health coach who will motivate you and help you reach your goals. You and your family will go through a clinical screening to determine eligibility for the program.

Hinge Health is building an inclusive community while also focusing on health equity in MSK care.

Musculoskeletal Care – Airrosti and Airrosti Remote Recovery

The Departments of the Treasury, Labor, and Health and Human Services (the Departments) have issued the Transparency in Coverage final rules (85 FR 72158) on November 12, 2020. The final rules require non-grandfathered group health plans and health insurance issuers in the individual and group markets (plans and issuers) to disclose certain pricing information. Under the final rules a plan or issuer must disclose in-network negotiated rates and billed and out-of-network allowed through machine-readable files posted on an internet website. Plans and issuers are required to make these files public for plan policy years beginning in 2022. This material is for information only.
