Supplemental AD&D Insurance

This benefit is paid if an accident causes a death. Benefits also are paid if a serious injury is suffered. Such benefits are paid as follows:

  • If, as the result of an accident, the person dies within 365 days of the accident, the beneficiary will receive AD&D benefits equal to 100% of the insurance amount—in addition to the life insurance benefit.
  • If the person injured is in an accident and suffers the loss of a limb and/or eyesight, the beneficiary will receive a portion of the full benefit amount, based on the extent of the loss, as indicated below:

LOSS:
Both hands or both feet or both eyes, or one hand and one foot, or one hand and one eye, or one foot and one eye

BENEFIT:
100% of insurance amount

LOSS:
One hand or one foot or one eye

BENEFIT:
50% of insurance amount

Who You Can Cover

Employee: One to five times your annual base salary (maximum benefit of $500,000)

Spouse or Domestic Partner: 50% of employee amount

Children: 15% of employee amount for each child
